BY CLICKING ON THE “ACCEPT” BUTTON BELOW, YOU ACKNOWLEDGE THAT YOU HAVE READ ALL OF THE TERMS AND CONDITIONS SET FORTH BELOW, UNDERSTAND ALL OF THE TERMS AND CONDITIONS OF THIS AGREEMENT, AND AGREE TO BE BOUND BY ALL OF THE TERMS AND CONDITIONS OF THIS AGREEMENT. I grant to Innovative Infusions, LLC, its successors and assigns, representatives and employees (collectively, “Innovative”) the right to use my comments submitted to Innovative, in whole or in part, for any lawful purpose including marketing, promoting and advertising Innovative. Innovative will own all rights to any such comments. I authorize Innovative to copyright, publish and use the comments in print and/or electronically. I agree that Innovative may use such comments with or without first and last name and for any lawful purpose. I will not, at any time, be compensated, in any way, and agree to waive any right to any compensation for making such comments.

I hereby release Innovative of any and all claims related to the use of the comments submitted to this forum by me.

I waive any rights I may have under the Health Insurance Portability and Accountability Act (“HIPAA”) relating to the use of any comments submitted by me.

This is a PUBLIC forum and should not be used to submit any private information. You hereby acknowledge and agree that the comments made or submitted by you may be made public and may be used in any lawful manner by Innovative.
